Integrated Care Management Program (ICMP)


Senior citizens, 60 years of age and older, or dependent adults, 18 years of age and older, experiencing difficulty in living independently, managing problems alone, and receiving needed assistance, are encouraged to contact a care manager at HSA. A qualified care manager will: conduct an in-home assessment; develop a care plan; arrange for services; and monitor the client’s progress. This comprehensive approach assures that each individual receives the appropriate services within a specified time frame. Topics usually overseen by Care Management include Social Security and Medi-Cal issues, in-home assistance, and eligibility for programs, amongst many others. Offered free of charge for residents in the following cities:

Multipurpose Senior Services Program (MSSP)

The Multipurpose Senior Services Program (MSSP) provides comprehensive care management for frail elderly clients who wish to remain in their community instead of being placed in a nursing facility. Clients eligible for the program must be 65 years of age or older, currently eligible for Medi-Cal, and certified or certifiable for placement in a nursing facility. Utilizing a comprehensive approach, each client will be monitored by a care manager and a public health nurse. MSSP is an excellent choice for clients wishing to remain in the community, but who have a need for substantial monitoring and support. The HSA MSSP program is provided in:

Alzheimer's Day Care Resource Center (ADCRC)

The Alzheimer's Day Care Resource Center (ACDRC) allows memory-impaired and/or socially isolated senior citizens to spend their day in a caring, supportive environment. ADCRC's goals are to delay or prevent nursing home placement, to provide a program meeting each client's needs, and to assist clients in achieving better management of incapacitating symptoms of dementia. Daily ADCRC activities include lunch and snacks, arts and crafts, and group therapy. English and Spanish support groups are also available for family members of ADCRC participants.

Caregiver Support Program


The Caregiver Support Program provides care management services to persons of any age caring for an elderly individual 60 and over. The caregiver must live in our catchment area detailed below. Care management provides assistance with service referrals, support, education, and counseling. Caregiver support groups are available in English and Spanish to caregivers of persons with Alzheimer’s or related dementias. Home-Based Care

Home-Based Care programs were created to provide seniors assistance in the home and include: In-Home Registry; homemaking; personal care; respite; and companionship. All services are available free of charge to seniors 60 and over in the following cities: Artesia; Bell; Bell Gardens; Bellflower; Cerritos; Commerce; Cudahy; Downey; Hawaiian Gardens; Hollydale; Huntington Park; Lakewood; Los Angeles (zip codes 90022, 90023, and 90063); Lynwood; Maywood; Paramount; South Gate; and Walnut Park.

• In-Home Registry
HSA's In-Home Registry matches workers with seniors, 60 and over, who need assistance in the home. The senior or his/her caregiver is responsible for hiring the worker of their choice. Workers provide housekeeping, meal preparation, personal care, transportation, and companionship, all in the senior's home. Workers' duties vary, depending on the senior's needs. HSA checks worker eligibility, references, fingerprints/background, and TB clearance. In addition to employment referrals, workers are also afforded the opportunity to participate in quarterly trainings conducted by professionals in the home care field. Pay for these services is arranged between the client and the worker.


• Homemaking
A home chore worker performs light housekeeping duties until permanent help can be established. The home chore worker is an HSA employee who will assist with laundry, cooking, vacuuming, etc. during a four-hour shift in the morning or afternoon.


• Personal Care
For a limited period, a certified nurse's assistant (CNA) assists with personal care until permanent help can be established. The CNA is an HSA employee who will assist with feeding, bathing, toileting, etc. during a four-hour shift in the morning or afternoon.

Food Distribution

Once a month, pre-bagged nonperishable food is distributed to low-income residents of Bell Gardens. Participants receive an ID card valid for one year, which must be presented the day of the distribution.

Congregate Meals

Active seniors, ages 60 and over, and their spouses can enjoy the friendship, activities, and nutrition that HSA's congregate meals sites offer. A hot, nutritionally-balanced meal is served to seniors five days a week for a minimal donation. Hot lunches served five days a week for a minimal donation. Social activities are also available at the sites. Please call the site nearest you to learn more about the congregate meals program.

Home Delivered Meals

The Home Delivered Meals program provides homebound seniors with a daily hot meal and a reassuring friendly visit. HSA drivers are trained to observe any danger signs and report them back to the agency. For many seniors, a home delivered meal increases their sense of independence and prevents premature institutionalization. Meal recipients also receive holiday gift baskets and other holiday treats. Meals are delivered in Bell Gardens, Bellflower, Cerritos, Commerce, Downey, Hawaiian Gardens, Lakewood, Long Beach, Lynwood, and Paramount.

Telephone Reassurance

Home Delivered Meals recipients that qualify for HSA's Telephone Reassurance program receive two telephone calls per week to ensure their safety and well-being. The Telephone Reassurance worker will report any unusual situation to the agency or the proper authorities.
