Champions for Change is a healthy initiative at Human Services Association that will help reduce the prevalence of obesity among low-income Supplemental Nutrition Assistance Program Education (SNAP-Ed) eligible populations.  This will be accomplished by providing nutrition education workshops, promoting physical activity and collaborating with community partners to create healthier environments for low-income individuals and families in Bell Gardens, Bell, Maywood, South Gate, Huntington Park, and Cudahy.

El programa La tarifa de bajos ingresos es fácil (LIFE) - anteriormente Rider Relief, proporciona cupones a pasajeros elegibles de Metro, Long Beach, Montebello, Norwalk, Foothill, Torrance, o LADOT que compran un pase semanal o mensual.

Los cupones de subsidio LIFE están disponibles para los residentes de las comunidades de Gateway, Valle de San Gabriel, y South Bay del Condado de Los Angeles; cuyos ingresos familiares caen por debajo de las siguientes pautas de ingresos.  LIFE Income Guidelines

Hay cuatro tipos de cupones LIFE disponibles; Regular, Colegio o Vocacional, Estudiante, y Mayor o Discapacitado. (Debe solicitar una tarjeta TAP regular o una tarjeta TAP de tarifa reducida para solicitar los cupones LIFE).

$ 24 de descuento - para usuarios regulares
$ 13 de descuento – para usuarios de universidad/vocacional
$ 10 de descuento - para usuarios estudiantiles de grados K-12
$ 8 de descuento - para pasajeros mayores o discapacitados

Los cupones de julio a diciembre de 2018 se distribuirán en los siguientes lugares, fechas, y horarios:

Todos los solicitantes deben volver a presentar una solicitud, por favor traiga una identificación con fotografía y tarjeta TAP para cada persona y un comprobante de bajos ingresos. Todas personas son bienvenidas desde el 25 de junio de 2018 durante horario normal de trabajo, NO SE NECESITA CITA.

Children’s Health Outreach, Enrollment, Utilization & Retention Services (CHOEUR)
The CHOEUR project is committed to educating and assisting families residing in the SPA 7 (Service Planning Area 7) region of the Los Angeles County with enrolling to no-cost or low-cost healthcare coverage programs. Our program provides application assistance for uninsured children and families who may be eligible for Medi-Cal, Medi-Cal Access Program (MCAP), Covered California, Kaiser Permanente Child Health Program, and we also provide referrals to Los Angeles County programs such as; Ability to Pay (ATP), My Health L.A, Child Delivery Program, and many others. Our staff is bilingual, trained, and fully committed to assist families with their health coverage needs.

The Low-Income Fare is Easy (LIFE) program – formerly Rider Relief, provides coupons to eligible Metro, Long Beach, Montebello, Norwalk, Foothill, Torrance, or LADOT Transit riders who purchase a weekly or monthly pass.

LIFE subsidy coupons are available to residents of the Gateway, San Gabriel Valley, and the South Bay Regions of Los Angeles County; whose household income falls below the following income guidelines.  LIFE Income Guidelines

There are four types of LIFE coupons available; Regular, College/Vocational, Students K-12, or Senior/Disabled. (You must have a Regular TAP card or Reduced Fare TAP card to apply for the LIFE coupons.)   

$24 off – for Regular riders who purchase a weekly or monthly pass
$13 off – for College/Vocational riders
$10 off– for student riders in grades K-12
$8 off – for for Senior or Disabled riders

July – December 2018 coupons will be distributed at the following locations, dates and times:

All applicants must reapply, please bring Photo ID and TAP card for each person, and proof of low income. Walk-ins are welcome as of June 25, 2018 during normal business hours, NO APPOINTMENT NEEDED.
