Job Description

JOB TITLE: MSSP Supervisor

DEPARTMENT: Senior Services

PROGRAM: MSSP

STATUS: Full-Time; Non-Exempt

REPORTS TO: MSSP Site Director

HOURS PER WEEK: 40

LOCATION: Monrovia, CA


Position Summary:

Under the supervision of the MSSP Site Director, the MSSP Supervisor provides direct supervision of Social Work Care Managers (SWCM) and Nurse Care Managers (NCM).


Duties and Responsibilities:

The major responsibilities of this position include, but are not limited to:

  • Provides direct supervision of SWCM & NCM.
  • Provide ongoing consultations and recommendations regarding participants and services.
  • Conduct annual performance evaluations.
  • Troubleshoot any issues with service provision with vendor, participants, caregivers and family.
  • Review and approve all participant care plans.
  • Review progress notes and conduct chart audits.
  • Conduct weekly team conference meetings with staff.
  • Oversight and authorization of purchases utilizing Waiver Services including verification that other resources have been exhausted.
  • Monitoring and tracking of waiver expenditures.
  • Ensure implementation of policies and procedures.
  • Ensure implementation of guidelines provided in the Site Manual.
  • Screen applicants and assign referrals as needed to ensure meeting monthly census goals.
  • Submit billing to the Medi-Cal as needed to support the team & program.

  • Knowledge of community resources, specifically related to the aging population.
  • Ability to display equanimity in difficult situations.
  • Ability to learn quickly in a fast-paced environment.


Education and Experience:

  • Master’s Degree in social work, psychology, nursing, counseling, gerontology, sociology or rehabilitation.
  • Two (2) years of direct experience working with the elderly


Licensing/Certification Requirements:

  • Occasional driving is required. Must provide own vehicle, valid California driver license and proof of full insurance coverage.


Other Requirements:

  • Negative tuberculosis screen
  • We require the selected candidate to undergo a criminal background check using the Live Scan fingerprinting method in compliance with applicable laws and regulations


Work Environment/Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job or that an employee encounters while performing the essential function of this job.

  • Work in moderate noise levels and non-smoking environment
  • Ability to occasionally lift and/or move up to 25 pounds independently
  • Frequently involves sitting, standing, walking, squatting, bending, and climbing stairs (as needed)
  • Occasionally may be required to stoop, kneel, crouch, or crawl
  • Must be able to speak and hear clearly to communicate effectively
  • Constantly operates a computer, keyboard, and other various office machinery
  • Majority of job duties are performed onsite; however, visits to different sites may be required

Compensation:

In accordance with California law, the expected pay range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
