Job Description
JOB TITLE: Public Health Nurse
DEPARTMENT: Senior Services
PROGRAM: M.S.S.P.
STATUS: Non-Exempt
REPORTS TO: M.S.S.P. Supervisor
HOURS PER WEEK: 20
POSITION SUMMARY:
Under the direction of the Supervisor Care Manager, the PHN is responsible for identifying the health limitations, which impeded independent living.
DUTIES AND RESPONSIBILITIES:
The major responsibilities of this position include, but are not limited to:
- Conduct in-depth assessment/reassessments covering medical, health, and rehabilitation concerns
- Certify level of care determinations based on health assessment findings
- Perform physical assessments, as necessary, and interpret clinical findings
- Work closely with Social Workers and Supervisors in developing care plans
- Conduct outreach and engagement in the community
- Provide feedback on appropriateness of waiver services/purchases for participants
- Participate in Care Plan review team meetings
- Provide consultation to Social Workers and Supervisor regarding interventions
- Perform case recording and documentation in a timely manner
- Conduct annual level of care determinations
- Assists and coordinates with a multidisciplinary team on enrolling new applicants
- Will carry a partial caseload as needed to support the program
- Related duties, as required
KNOWLEDGE, SKILLS AND ABILITIES:
- Must enjoy working with senior citizens and have experience in case management, particularly for the geriatric population
- Knowledgeable in community resources and other area social service agencies
- Strong documentation and organizational skills required
- Must be comfortable with providing services in the home.
- Able to coordinate service delivery with diverse service providers
- Thorough knowledge of medical conditions, physical limitations, and medications as they pertain to the geriatric population
- Strong attention to detail and time management skills
EDUCATION AND EXPERIENCE:
- Bachelor’s degree in nursing (public health, health education, health administration, gerontology, or equivalent)
- Two years’ experience, public health, and/or geriatrics, preferred
LICENSING/CERTIFICATION REQUIREMENTS:
- Daily driving is required. Must provide own vehicle, valid California driver license and proof of full insurance coverage.
- First Aid certification, CPR certification.
- Negative tuberculosis screen (required)
- We require the selected candidate to undergo a Live Scan (background check).
- Must maintain active registration and license with the Board of Registered Nursing
OTHER REQUIREMENTS:
- Bilingual (English/Spanish) strongly preferred
