Supportive Services Program
The Supportive Services Program (SSP) helps elders obtain services that enable them to safely remain at home with a measure of independence and dignity.
Offered Services
SSP services include:
- Ongoing Care Management
- Assessments
- Client Advocacy
- Service Coordination
- Referrals and Resources
Program Eligibility
To be considered eligible for SSP, clients must be:
- 60 years or older
- Experiencing difficulty with daily living activities
- Homebound
- Willing to participate in Care Management
This program is offered for the following cities: Arcadia, Avalon, Azusa, Baldwin Park, Bell, Bell Gardens, Bellflower, Bradbury, Cerritos, Cypress, Claremont, Covina, Commerce, Cudahy, Diamond Bar, Downey, Duarte, East Los Angeles, Glendora, Hawaiian Gardens, Huntington Park, Hacienda Heights, Lakewood, La Puente, La Verne, La Mirada, La Habra, Maywood, Monrovia, Pico Rivera, Paramount, Pasadena, Pomona, and South Gate.
